29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing, or blocking of machines or equipment from energy sources: a) Cross Member line; On or about October 7, 2015 and at times prior thereto, the employer exposed employees to the hazardous energies present at the robot welder (including electrical, pneumatic, and gas) in that the employer did not provide locks, tags, or other hardware for employees to use to lock out the machine so it could not be accidentally started when employees are required to enter the cage to access the robot and clean the welding tips. b) LFA line; On or about October 7, 2015 and at times prior thereto, the employer exposed employees to the hazardous energies present at the robot welder (including electrical, pneumatic, and water) in that the employer did not provide locks, tags, or other hardware for employees to use to lock out the machine so it could not be accidentally started when employees are required to enter the cage to access the robot and change the welding tips.
