Randy Manges DBA Manges Construction worksite located at 102 Empire St., Montpelier, Ohio: On or about August 28, 2014, the employer did not ensure employees were protected from electrical shock while operating the Genie S60 aerial lift in close proximity to the energized main electric service to the structure. Employees were installing steel siding near the single phase 400/120/240 volt service line.

Randy Manges DBA Manges Construction worksite located at 102 Empire St., Montpelier, Ohio: On or about August 28, 2014, the employer did not ensure employees were wearing fall protection while operating the Genie S-60 Aerial Lift. The exposed employees were installing metal siding on the south side of the commercial structure from the aforementioned aerial lift.

Employees were near the edge of the roof structure preparing to remove the gutter with no fall protection.
