On or about August 28, 2014, in the tank shop, an electrical cord on the bottom of the 220 volt electrical panel was not protected from direct pull to the terminal and/or internal wiring by a strain relief mechanism.  The outer sheathing was pulled away from the terminal end exposing the internal wiring and exposing employees to the hazard of electrical shock.
