29 CFR 1926.451(b)(1): 29 CFR 1926.450(b)(1):  Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports:  On or about August 28, 2014, and at times prior thereto, the employer did not ensure that all the tiers of the three tier tubular welded frame scaffold system were fully planked, exposing employees to a fall hazard of approximately 21 feet.

29 CFR 1926.451(e)(1): Cross braces were used as a means of access and egress:  On or about August 28, 2014, and at times prior thereto, employees accessed the scaffolding by climbing the cross braces where the employer did not ensure that there were ladders installed on the three tier tubular welded frame scaffolding, exposing the employees to a fall hazard of approximately 21 feet.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  On or about August 28, 2014, and at times prior thereto, the employer did not ensure that guardrails were installed on the second and third levels of a two tier tubular welded frame scaffold system, exposing the employees on the scaffold to a fall hazard of up to approximately 21 feet.

29 CFR 1926.454(b): The employer did not have each employee involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work in question.  On or about August 28, 2014, and at times prior thereto, the employees for the company were not trained by a competent person to recognize the hazards associated with the use of a scaffold, exposing the employees to the fall hazard of approximately 21 feet.
