29 CFR 1926.451(e)(1): Cross braces were used as a means of access and egress: On or about August 28, 2014, and at times prior thereto, employees accessed the scaffolding by climbing the cross braces where the employer did not ensure that there were ladders installed on the three tier tubular welded frame scaffolding, exposing the employees to a fall hazard of approximately 21 feet.
