29 CFR 1917.112(b)(1): Guardrails were not provided at locations where employees were exposed to floor or wall openings or waterside edges, including bridges or gangway-like structures leading to pilings or vessel mooring or berthing installations, which presented a hazard of falling more than 4 feet or into the water: (a) At the barge terminal, the guardrail for the cell and barge loading area presented a hazard of falling in that the railing did not extend towards the sea wall leaving an open area of approximately 50 feet where 20 feet exposed employees to falls of more than 4 feet or into the water that was about 8-10 feet deep.

29 CFR 1917.152(d)(1)(i): Compressed gas cylinders for gas welding and cutting operations did not have valve protection caps in place: (a) In the maintenance shop, an oxygen cylinder did not have a valve protection cap and cylinder was exposed to the potential of having the valve sheared off as there were tools stored above the cylinder.

29 CFR 1917.152(d)(1)(xvi): Oxygen cylinders were not separated from fuel gas cylinders and combustible materials by either a minimum distance of 20 feet (6.1 m) or a barrier having a fire-resistance rating of 30 minutes: (a) In the maintenance shop, an oxygen cylinder was stored next to a flammable cabinet with combustible materials and oils stored within 20 feet of the oxygen cylinder.

29 CFR 1917.120(b)(1): Riser height was not 6 to 7.5 inches (15.24 to 19.05 cm), stair width was not a minimum of 22 inches (55.88 cm) between vertical barriers, and tread depth was not a minimum of 12±2 inches (30.48±5.08 cm): (a) At the barge terminal, a fixed step was used to access the conveyor walkway with the riser height approximately 11 inches from the ground to the step and from the step to the walkway the riser height was approximately 9 inches. There was no handrail or grab bar to assist with access onto the walkway creating a trip and fall hazard for employees.
