29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:        a) On or about 8/19/14, inside the west end of the Saks Store under construction at the Fashion Outlets of Niagara Falls, 1900 Military Rd., Niagara Falls, NY:  A Super Vac fan was used to provide ventilation in an area being painted. The fan was connected to 120 volt-ac power via a series of extension cords, the first of which was plugged into a receptacle in a temporary power distribution center. The plug on this first extension cord lacked a ground prong.         NO ABATEMENT CERTIFICATION REQUIRED
