On the north/east side of the building where employees were working from a pump jack scaffold, the footings were not placed on level, sound, rigid ground capable of supporting the scaffold, thereby exposing the employees to a 15 foot fall hazard.

29 CFR 1926.451(c)(2)(iii): Unstable objects shall not be used as working platforms.    a. On the north/east side of the building where employees were working from a pump jack scaffold, a 2 inch x 12 inch x 7 foot 6 inch board was attached to the end of the platform to extend the length of the scaffold out approximately 2 additional feet, thereby exposing the employees to a 15 foot fall hazard.

Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews.    a. On the north side of the building where employees were working from the platform of a pump jack scaffold, end rails were not installed, thereby exposing employees to a 13 foot fall hazard.    b. On the north east side of the building where employees were working from the platform of a pump jack scaffold, end rails were not installed, thereby exposing employees to a 15 foot fall hazard.

On the north side of the building where employees were working from a pump jack scaffold, the safety screen was not installed along the entire space between the up rights, thereby exposing employees to a 13 foot fall hazard.  b. 29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. Exception: When the employer can demonstrate that it is infeasible or creates a greater hazard to use these systems, the employer shall develop and implement a fall protection plan which meets the requirements of paragraph (k) of 1926.502. Note: There is a presumption that it is feasible and will not create a greater hazard to implement at least one of the above-listed fall protection systems. Accordingly, the employer has the burden of establishing that it is appropriate to implement a fall protection plan which complies with 1926.502(k) for a particular workplace situation, in lieu of implementing any of those systems.  a. On the roof of the 1st floor entrance, north side, where employees were installing siding, the employees were not protected from falls by guardrail systems, safety net system, or personal fall arrest systems, thereby being exposed to a 13 foot fall hazard.
