THE FOLLOWING ARE BEING GROUPED: 29 CFR 1926.405(a)(2)(ii)(J): Extension cord sets used with portable electric tools and appliances were not designed for hard or extra-hard usage: (a) Jobsite - On or about August 20, 2014 employees using a Milwaukee electric socket-driver/impact wrench to tighten bolts connecting a cut-off valve to sections of 8-inch diameter black-iron pipe being laid down in an excavation were potentially exposed to electric shock due to the absence of outer insulation for 2-3 inches in two places along the length of a round orange AWG 16/3 extension cord which was supplying electric current to the tool.

THIS VIOLATION IS BEING GROUPED WITH THE PREVIOUS STANDARD: 29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: (a) Jobsite - On or about August 20, 2014 employees using a Milwaukee electric socket-driver/impact wrench to tighten bolts connecting a cut-off valve to sections of 8-inch diameter black-iron pipe down in an excavation were potentially exposed to electric shock due to the lack of strain-relief at the female plug of a round orange AWG 16/3 extension cord.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: (a) Jobsite - On or about August 20, 2014 employees setting 8-inch diameter x 18 foot long joints of black-iron water pipe and a valve in a 5-6 foot wide (at top) x 4.4 to 4.6 foot deep x 54 foot long excavation were exposed to possible cave-ins and crushing due to the absence of 1.5:1 or 34 degree sloping of the east and west sides of the excavation. Employees were not being protected by any other cave-in protective systems.
