29 CFR 1926.451(g)(4)(iii): When midrails, screens, mesh, intermediate vertical members, solid panels, or equivalent structural members are used, they were not installed between the top edge of the guardrail system and the scaffold platform. (a). At the Wekiva Springs Publix shopping center an employee elevated in a scissor lift did not have a completed guardrail system emplace to protect him from a 7 foot 4 inch fall hazard, on or about August 29, 2014.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: (a) At the Wekiva Springs Publix shopping center, the employee elevated approximately 10 feet in the basket of an articulating boom aerial lift was not using a safety belt or harness with a lanyard attached to the boom or basket which would protect him from being ejected from the elevated basket while traveling, on or about August 29, 2014.
