29 CFR 1926.28(a): The employer did not require the wearing of appropriate personal protective equipment in all operations where there is an exposure to hazardous conditions. (a) At the site of the proposed Khalaf Plaza in West Point, MS - On or about August 22, 2014 an employee operating the Caterpillar CS-563-E articulating front-end Roller Compactor was not wearing the provided seat belt while compacting fill dirt atop sections of freshly laid 24 inch diameter concrete drainage pipe.
