29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports as follows: (a) Backside of the building - On or about August 29, 2014 an employee was exposed to a fall hazard due to a scaffold not being fully decked or planked. Only one deck was installed on the scaffold resulting in a 36 inch gap where employees could fall 15 feet to the ground.

29 CFR 1926.451(g)(4)(i): Guardrail systems shall be installed along all open sides and ends of platforms. Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews. (a) Backside of the building - On or about August 29, 2014 an employee was exposed to a fall hazard while working on a scaffold 15 feet above the ground with incomplete guardrails. Only one guardrail was installed.

29 CFR 1926.452(c)(2): Frames and panels shall be braced by cross, horizontal, or diagonal braces, or combination thereof, which secure vertical members together laterally. The cross braces shall be of such length as will automatically square and align vertical members so that the erected scaffold is always plumb, level, and square. All brace connections shall be secured. (a) Backside of the building - On or about August 29, 2014 an employee was exposed to a fall hazard while working on a scaffold that was missing two sets cross-braces on the backside.

29 CFR 1926.454(b)(1): The nature of scaffold hazards; (a) N & W Construction Company, Inc. - On or about August 29, 2014 employees were not trained on how to assemble, disassemble, operate, and inspect scaffolds. Employees demonstrated an inability to properly build and inspect scaffolds. The scaffold had the following discrepancies: 1) Incomplete guardrails were installed 2) Cross-braces were missing 3) The platform was not fully decked or planked.
