29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. The employer does not protect employees from falling when working 6 feet or more above a lower level. This violation was observed on or about August 25, 2014, at the job site where an employee was exposed to a 8 foot, 8 inch fall hazard when performing masonry work without a fall protection system. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees are protected from falling when exposed to falls 6 feet or more above a lower level.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: The employer does not ensure that flexible cords are connected to devices and fittings so that strain relief is provided. This violation was observed on or about August 25, 2014, at the job site where an employee was operating a radial saw that was plugged into an electrical cord with outer sheath damage from lack of strain relief. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure flexible cords are connected to devices and fittings so that strain relief is provided.
