29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch Maintenance Shop/Welding Shop - A DeWalt and Delta double wheeled bench grinders did not have properly adjusted tongue guards on both the left and rights sides to 1/4" as required; the DeWalt grinder had a measured 1/2" gap on both sides and the Delta grinder had measured gaps of 3/4" on the left side and 1/2" on the right side.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): Maintenance Shop - The power cord and plug of a Milwaukee angle grinder was damaged through to the insulation in multiple areas.
