29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables used for temporary wiring were not protected from damage.  On 9/2/14, at the Arbors of Grayson jobsite, employees were using an extension cord that had been laid across a frequently used roadway and was exposed to vehicular traffic to power tool chargers and music boxes at the site. The cord had exterior damage with exposed inner conductors.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On 9/2/14, at the Arbors of Grayson jobsite, employees were exposed to fall hazards greater than 10 feet as they worked on the second story of the site installing HVAC duct. No fall protection was used to protect employees from falling to lower levels.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards.  On 9/2/14, at the Arbors of Grayson jobsite, employees were exposed to fall hazards greater than 10 feet. The employer did not provide a training program to employees so as to educate them on identifying and eliminating fall hazards encountered on site.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.   On 9/2/14, at the Arbors of Grayson subdivision jobsite, employees were using a portable A-frame ladder to access second story levels of a home under construction. The Ladder did not extend at least 3' above the upper landing surface being accessed and was approximately 2 feet below the access surface.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
