29 CFR 1926.451(g)(2): The employer did not have a competent person determine the feasibility and safety of providing fall protection for employees erecting or dismantling supported scaffolds:    (a) Jobsite, Lot 227, The Gateway at Summerset, Pittsburgh, PA:  On or about 09/02/14 the employer did not have a competent person determine the feasibility of providing fall protection for employees during erecting and dismantling of the supported scaffold.  An employee was erecting a scaffold and was working at heights up to approximately 25 feet above the ground and no fall protection systems were in place to prevent falls.  It was not infeasible to utilize a personal fall arrest system during the erection of the scaffold in that anchor points existed for a system to be installed and used.

29 CFR 1926.602(d): The employer did not certify that each operator had been trained and evaluated as required by paragraph 29 CFR 1910.178(l):    (a) Jobsite, Lot 227, The Gateway at Summerset, Pittsburgh, PA:  On or about 09/02/14 the employer did not certify that the operator of the Gehl powered industrial truck had received training and was evaluated to operate the equipment.
