29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees: (a) On or about August 21, 2014, for employees installing a water main from within a trench that was approximately 11 feet deep where employees exited the trench box through the unprotected area of the excavation at the work site located near East Street in Grenora, North Dakota. Note: This company was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard which was contained in OSHA Inspection Number 314700774 , Citation 1, Item 1, and was affirmed as a final order on October 4, 2010, with respect to a workplace located at RNT Water Supply in Ray, North Dakota.
