The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head. (a) Roof of the house at Lot # 5, 2208 Bonniewood Dr. SW, Marietta, GA: An employee was exposed to fall hazards while improperly using a harness and carabineers as a positioning belt with a rope grab located at the wearer's waist. The employee was on a 12:12 pitched roof at a height of approximately 21 feet from the eave to the lower level.
