29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: a) On or about August 22, 2014; egress from a trench: The employer failed to ensure employees conducting repair work to an AT&T communication cable had proper points of egress no more than 25 feet from their work location. Employees working at the West end were about 25 feet or more away from a stepped down ramp at the East end resulting in them climbed out of the almost vertical West end with no ladder, slope, ramp, or stairway.
