29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:    a. The lockout program was not inspected annually including documentation showing that all machine specific procedures were included in the inspection with dates and identification of the person conducting the inspection.     Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citation(s).

29 CFR 1910.147(c)(7)(iii)(B): Additional retraining was not provided whenever a periodic inspection under 29 CFR 1910.147(c)(6) revealed that there were deviations from or inadequacies in the employee's knowledge or use of the energy control procedures  a. Re-training on lockout/tagout was not provided after an injury on 11-15-13 resulting in a finger tip amputation, where the machine was not locked out during maintenance work.  Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citation(s).

29 CFR 1910.178(q)(1): Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel:    a. Locator pins were not present on two propane forklifts resulting in incorrect orientation of the propane tank on the lift (the propane tank on another lift was not oriented correctly on the lift even though a locator pin was present).
