29 CFR 1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and walkway shall be at least 18 inches (46 cm) wide. Southeast corner of building, On or about August 26, 2014, an employee was exposed to a fall hazard greater than 10 feet to the dirt ground below while working on the building's roof flashing while standing on an approximately 9 inch work platform.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: Southeast corner of building, On or about August 26, 2014, an employee was exposed to a fall hazard greater than 10 feet to the dirt ground below while working on the building's roof flashing while standing on the outrigger section of the scaffolding which did not have guardrails installed.
