29 CFR 1926.451(f)(6): Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6): a) Roof - An employee installing conduit for a new service was exposed to an electrical hazard from working within approximately one (1) foot of a live service line, on or about August 27, 2014. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement. Note: Specify distances as indicated in table in paragraph (f)(6) of this section.

29 CFR 1926.451(g)(1)(iii): Each employee on a crawling board (chicken ladder) was not protected by a personal fall arrest system, a guardrail system (with minimum 200 pound toprail capacity), or by a three-fourth inch (1.9 cm) diameter grabline or equivalent handhold securely fastened beside each crawling board: a) Roof - An employee installing a new service line was exposed to a fall hazard of approximately ten (10) feet three (3) inches without the use of a personal fall arrest system, guardrail system or a grabline, on or about August 27, 2014. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
