29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy.  (a)  Employees were exposed to hazardous energy as there were no machine specific lockout tagout procedures that detailed the location where on the machine to lock out different power sources for machinery that was hardwired in, had more than one power, or has a disconnect not located on the machine, such as, but not limited to the Komori Press, and the Webtron Press.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a)  Maintenance Shop:  Employees were exposed to point of operation hazards as the unused portion of the blade for the Enco Metal Cutting Band Saw was not guarded.  (b) Press Area:  An employee was exposed to the ingoing nip points from the rollers on the Stevens Press as the top guards and the front guards of each section of the press was left open while the press was being operated.  (c) Press Area: Employees were exposed to pinch points as the brake portion at the input end for the Webtron Press had openings in the guard on the top and side of the mesh guard.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  (a) Maintenance Shop: Employees were exposed the hazard of flying debris as the Dayton 10 inch Bench Grinder there was no adjustable tongue guards to adjust to the decreasing periphery of the wheel on both wheels.  (b) Maintenance Shop:  Employees were exposed the hazard of flying debris as the 6 inch Craftsman Bench Grinder the tongue guard was adjusted at greater than 1/4 inch.
