29 CFR 1910.22(b)(2): Permanent aisles and passageways were not appropriately marked:    On or about September 3, 2014 and times prior thereto, employees of Alliance Parts Warehouse, LLC received and distributed auto parts in the warehouse located at or near 600 Fiber Optics Drive in North Little Rock, Arkansas.  Employees operated powered industrial vehicles inside of the facility, the employer did not have permanent aisles and passageways appropriately marked, exposing employees to struck-by hazards.

29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:Note:  Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard.    On or about September 3, 2014 and times prior thereto, employees of Alliance Parts Warehouse stored and distributed auto parts at the warehouse located at or near 600 Fiber Optics Drive in North Little Rock, Arkansas.  Employees performed sorting activities on the third floor where an open-sided platform was observed without guarding approximately 20 feet from ground level, exposing employees to fall hazards.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:    On or about September 3, 2014 and times prior thereto, employees of Alliance Parts Warehouse, LLC received and distributed auto parts in the warehouse located at or near 600 Fiber Optics Drive in North Little Rock, Arkansas.  The employer had a flexible extension cord used as a substitute for fixed wiring near the digital scale, exposing employees to fire hazards.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:    On or about September 3, 2014 and times prior thereto, employees of Alliance Parts Warehouse, LLC received and distributed auto parts in the warehouse located at or near 600 Fiber Optics Drive in North Little Rock, Arkansas.  The employer had used a charging unit for a golf cart which was observed without strain relief.  The sheathing had pulled from the plug exposing insulated conductors.
