29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: (a) Jobsite, 2055 North Main Street, Washington, PA: On or about September 3, 2014, two employees were working on a roof (300 feet by 80 feet) and they were not protected from falling nor was there a warning line system and safety monitoring being used. The fall hazards to the employees ranged from about 18 feet to about 22 feet.

29 CFR 1926.503(c): When the employer has reason to believe that any affected employee who has already been trained does not have the understanding and skill required by paragraph (a) of this section, the employer is to retrain each such employee such as inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding of skill: (a) Jobsite, 2055 North Main Street, Washington, PA: On or about September 3, 2014, two employees were working on a roof (300 feet by 80 feet) and the two employees need to be retained so that they retain the necessary skills to minimize fall hazards when working on a roof.
