The A-Team Ship Repair and Fabrication Corporation, General Dynamics NASCO Norfolk, USS Carter Hall, Tank 6-35-2-W: On or about 08/22/14, an employee was injured by the unguarded blades of the Americ VAF 3000 ventilation fan. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
