29 CFR 1910.212(a)(5): Fan blade guard(s) were not provided where the periphery of the blades was less than seven feet from the floor or working level:  a.)  The A-Team Ship Repair and Fabrication Corporation, General Dynamics NASCO Norfolk, USS Carter Hall, Tank 6-35-2-W:  On or about 08/22/14, an employee was injured by the unguarded blades of the Americ VAF 3000 ventilation fan.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1915.56(b)(2): Welding cable was not free from repair or splices for a minimum distance of 10 feet from the cable end to which the electrode holder was connected.:  a.)  The A-Team Ship Repair and Fabrication Corporation, General Dynamics NASCO Norfolk, USS Carter Hall, Tank 6-67-1-W:  On or about 8/28/14, an employee was using a Flux Core welding lead that had a repair 32" from the electrode holder.   Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
