The A-Team Ship Repair and Fabrication Corporation, General Dynamics NASCO Norfolk, USS Carter Hall, Tank 6-35-2-W: On or about 08/22/14, an employee was injured by the unguarded blades of the Americ VAF 3000 ventilation fan. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

The A-Team Ship Repair and Fabrication Corporation, General Dynamics NASCO Norfolk, USS Carter Hall, Tank 6-67-1-W: On or about 8/28/14, an employee was using a Flux Core welding lead that had a repair 32" from the electrode holder. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
