29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: a- 2400 Route 130, North, Dayton, NJ - A floor opening, approx. 68" x 68" on the mezzanine level was not guarded by guardrails, covers or by a personal fall arrest system, to prevent employee(s) from falling approx. 16 ft. to the floor below. Condition noted on or about 9/3/14.
