29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: On or about September 4, 2014, at the above addressed site, an employee operating an aerial lift while exposed to fall hazards was not provided with a training program that would enable him to recognize the hazards associated with using that lift and in the procedures to protect him.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: On or about September 4, 2014, at the above addressed site, an employee performing stucco work from the basket of an aerial lift was exposed to a fall hazard of approximately 13 feet 6 inches while not wearing a safety harness with a lanyard attached to the boom or basket of that lift. I/X DESIGN, INC. WAS PREVIOSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.453(b)(2)(v), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 958923, CITATION NUMBER 1, ITEM NUMBER 1 AND WAS AFFIRMED AS FINAL ORDER ON 07/08/2014, WITH RESPECT TO A WORKPLACE LOCATED AT 1807 PALM BEACH LAKES BOULEVARD IN WEST PALM BEACH, FLORIDA.
