29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net system, personal fall arrest system, or other approved alternative fall protection system:      (a)  Alfredo Adame at 4885 S Quebec St, Denver, CO 80237:  On or before September 3, 2014 the exposing employer did not ensure employees were protected from injury, in that ten employees removing and installing shingles on a two-story hotel were not utilizing a fall protection system.  The pitch of the roof was approximately 6-in-12.  This condition exposed employees to a fall of approximately 18 feet.
