29 CFR 1910.147(c)(4)(i): No procedures was developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section.     a) Reliance Vitamin., Edison, NJ- Lockout procedures were not developed where employee(s) are required to perform maintenance and in service adjustments exposing employees to amputation hazards on machines such as but not limited to the Stokes Tablet Press in Room # 5.    b) The Index K120i  machine needed to be energized  while employees where cleaning the machine in order to jog the machine at a certain speed to clean the compartments. The employer did not developed or document a program for the control of potentially hazardous energy exposing employees to amputation hazards.       Observed on or about on 9/04/2014

No guards were provided to protect the employees from ingoing nip points and rotating parts exposing the employees from amputation hazards.    2) The Index K120i was not guarded from the right side of the machine where employees are instructed to fill the hopper.
