29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: a) Exterior of building, 31 Towne Center Drive, Leechburg, PA 15656: Observed on or about September 03, 2014, employees working from the basked of a JLG aerial lift with an articulating boom were not protected from falling by the use of personal fall arrest systems with a lanyard attached to the boom or basket. The employees were exposed to a fall hazard of approximately 17 feet to the ground below.

29 CFR 1926.454(a): The employer did not have each employee who performs work while on a scaffold(s) trained by a person qualified in the subject matter to recognize those hazards associated with the type of scaffold(s) being used and to understand the procedure(s) to control or minimize those hazards: a) Exterior of building, 31 Towne Center Drive, Leechburg, PA 15656: Observed on or about September 03, 2014, the employer did not provide training to employees working from the basked of a JLG aerial lift. The employer did not ensure employees working on JLG lift were trained by a person qualified in the subject matter to recognize the hazards associated with a JLG lift and the procedures to control or minimize these hazards. The employees were exposed to a fall hazard of approximately 17 feet to the ground below.
