Hardinger Transfer Company/West 18th Street Erie, PA/Truck Maintenance Garage. - On or about 8/28/14, employees were exposed to crush and impact hazards form powered industrial trucks that were not equipped with proper safety equipment/operator restraint systems.  Clark Forklift (Green Colored) was not equipped with an operator restraint system (i.e. seat belt).  Employees operated the forklift for activities that included, but were not limited to lifting/maneuvering items around the truck maintenance garage area.

Hardinger Transfer Company/West 18th Street Erie, PA/Crate Shop:  On or about 8/28/14, employees were potentially exposed to a fire hazard from the accumulation of wood dust in around the chop saw,  DeWalt Miter Saw, panel saw, and stacks of wood around/under the saws.  The saw dust was accumulating in piles in and around sections of the crate area and saws.  In addition, a pile of saw dust exceeding over 6-inches was accumulating inside the cabinet/housing of the chop saw.

Capacity, operation, and maintenance instruction plates, tags, or decals were not changed on the forklifts to reflect the specialized attachments.  In addition, authorization to utilize the attachments was not obtained from the forklift manufacturer. The attachments were utilized on forklifts that included but were not limited to the Kalmar forklifts.
