29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) Site -  On or about 08/28/14 and at times prior thereto employees were building and erecting the external walls on top of the second floor. The floor border was not protected by guardrails, and fall protection equipment was not in use. Employees were exposed to  10 ft. 8.0 in. falls to ground level.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition documentation demonstrating that abatement is complete must be included with your certification

29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, was not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system.  a) Site -  On or about 08/28/14 and at times prior thereto employees were performing carpentry work on top of the second floor.  The window opening on the right side of the building was not protected by guardrails, exposing workers to 10 ft. 8.0 in. falls to ground level.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition documentation demonstrating that abatement is complete must be included with your certification

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a) Site -  On or about 08/28/14 and at times prior thereto employees were building and erecting the external walls on top of the second floor. The portable Werner aluminum ladder reached 9.0 ft. above ground level, and did not extend 3.0 feet above the landing surface (situated at 10.0 ft. 8.0 in. above ground level).   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition documentation demonstrating that abatement is complete must be included with your certification

29 CFR 1926.1060(a)(1): 1926.1060(a)(1):  The employer did not ensure that each employee was trained by a competent person which would enable each employee to recognize hazards related to ladders and stairs, and the procedures to follow to minimize those hazards.  a) Site -  On or about 08/28/14 and at times prior thereto employees were building and erecting the external walls on top of the second floor. The portable Werner aluminum ladder reached 9.0 ft. above ground level, and did not extend 3.0 feet above the landing surface (situated at 10.0 ft. 8.0 in. above ground level). The employer did not ensure that workers were trained by a competent person in the safe use of ladders, and in the recognition of hazards associated with ladders.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition documentation demonstrating that abatement is complete must be included with your certification
