29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven -7 feet or less above floors or platforms were not enclosed: On or about September 3, 2014, and at times prior thereto, at the Formulation area a sprocket wheel and chain system was observed approximately 3-ft. from the floor without guards provided to prevent employees contacting the sprocket wheel and chain system while in operation. Employees were exposed to caught in and amputation hazards.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: On or about September 3, 2014, and at times prior thereto, at the battery charging area employees may be exposed to battery acid, an eyewash capable of providing 15 minutes of flushing was not available. Employees were exposed to eye injury hazards.
