29 CFR 1910.177(d)(4)(i): The employer did not furnish an air line assembly which included a clip-on chuck for inflating tires on rim wheels:    On or about September 3, 2014, and at times prior thereto, at the Lendel Vines Trucking facility, located at 5710 Alma Highway, in Van Buren, Arkansas, employees were required to inflate large truck tires, with an air line that did not include a clip on chuck for inflating the tires. These conditions exposed the employees to the hazard of being struck by the tire rims.

29 CFR 1910.177(d)(4)(ii): The employer did not furnish an air line assembly which included an in-line valve with a pressure gauge or a presettable regulator for inflating tires on rim wheels:   On or about September 3, 2014, and at times prior thereto, at the Lendel Vines Trucking Company's facility, located at 5710 Alma Highway, in Van Buren, Arkansas, employees were required to inflate large truck tires, with an air line that did not include an in-line valve or pre-settable regulator. These conditions exposed the employees to the hazard of being struck by the tire rims.

29 CFR 1910.177(d)(3)(ii): A rim wheel restraining device or barrier was not capable of preventing rim wheel components from being thrown outside or beyond the device or barrier for any rim wheel positioned within or behind the device:    On or about September 3, 2014, and at times prior thereto, at the Lendel Vines Trucking Company's facility, located at 5710 Alma Highway, in Van Buren, Arkansas, employees were required to inflate large truck tires, in a shop built cage that had an opening large enough to allow the rim to be thrown outside the cage. These conditions exposed the employees to the hazard of being struck by the tire rims.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:     On or about September 3, 2014, at the Lendel Vines Trucking Company's facility located at 5710 Alma Highway in Van Buren, Arkansas, employees cleaning tanker trailers and performing maintenance on trucks were using chemicals such as but not limited to degreaser and motor oil, and the employer had not developed a written hazard communication program. The employer did not provide material safety data sheets or any training in the proper handling of the chemicals. These conditions exposed the employees to the hazard of coming in contact with the chemicals.
