29 CFR 1926.502(d): The employer used body belts as part of a personal fall arrest system: Note:  The use of a body belt in a positioning device system is acceptable and is regulated under paragraph (e) of this section.     a) Roof of residence located at Lot 58H1 in the Arbor Springs subdivision, Newnan Georgia: Two employees, installing asphalt impregnated roofing felt, were not wearing personal fall arrest systems. Both employees were wearing mountaineering positioning belts which were attached to a lifeline via a lanyard and a rope grab. The employees were exposed to fall hazards of 13 and 24 feet.       Sirilo Silverio-Chino was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.502(d)(7), which was contained in OSHA inspection number 977264, citation number 01, item number 03 and was affirmed as a final order on August 08, 2014, with respect to a workplace located at 118 reflection Point, Fayetteville, Georgia.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:     a) Roof of residence located at Lot 58H1 in the Arbor Springs subdivision, Newnan Georgia. Employees were exposed to fall hazards of approximately 13-feet while mounting and dismounting an aluminum extension ladder. The side rails of the ladder did not extend at least 3-feet above the upper landing of two steep pitched, 10 in 12, unprotected roofs.      Sirilo Silverio-Chino was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 977264, citation number o1, item number 04 and was affirmed as a final order on August 08, 2014, with respect to a workplace located at 118 Reflection Point, Fayetteville, Georgia.
