29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person (i.e., a person who is capable of identifying exist:  On or about 09/04/14 located at 3995 Spring Ridge, Dr., Cumming, GA:  The employer did not perform frequent and regular inspections where employees were installing a vent on the roof without fall protection and no window guards.

29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached) where the outside bottom edge of the wall opening is 6 feet or more above lower levels and the inside bottom edge of the wall opening:   On or about 09/04/14 located at 3995 Spring Ridge Dr. Cumming, GA:  At the job site, the window wall openings were not protected, no window guardrails were in place, to prevent the employees from falling out the window opening to the ground lower level which was 12.5 feet. An employee was climbing in and out of the window to access the roof.

29 CFR 1926.501(b)(15): Each employee on a walking/working surface 6 feet (1.8 meters) or more above lower levels was not protected from falling by a guardrail system, safety net system, or personal fall arrest system:   On or about 09/04/14 located at 3995 Spring Ridge Dr, Cumming, GA:  Employee installing a roof vent for the AC on the roof while working from an unprotected steep 6 / 12 pitch roof was exposed to fall hazards of approximately 25.6 feet.
