Employees were working on the second floor of a residential home installing plywood decking. The decking to ground height measured approximately 11 feet and the employees had no fall protection system in place.

Employees were working on the second floor installing decking while reinforcing steel was protruding unguarded from the concrete blocks. The reinforcing steel measured approximately 30 inches in height and were located along all the perimeter walls and floor openings.
