Employees were not provided with the information contained in Appendix D in that the employees did not read and heed all instructions provided by the manufacturer and did not know what atmosphere the cartridge in use was manufactured to protect against.

Employees were using a respirator to spray sealant on the new sheds. Employer did not establish a written program that contained a medical certification of employees using the respirators and the proper storage of the respirator.
