This violation occurred on September 2, 2014, at a job site located at 1300 N. Country Club Road, Garland, Texas; where the employer did not ensure an employee hanging wire on a utility pole maintained a minimum approach distance with an overhead power line rated at 7.2 kVolts.

29 CFR 1904.39(a): Within eight (8) hours after the death of any employee from a work-related incident, the employer did not orally report the fatality by telephone, in person to the Area Office of the Occupational Safety and Health Administration (OSHA), U.S. Department of Labor, that is nearest to the site of the incident, or by calling the OSHA toll-free central telephone number, 1-800-321-OSHA (1-800-321-6742): This violation occurred on or about September 02, 2014, when the employer did not report to OSHA within eight (8) hours a fatality as the result of a workplace incident that occurred at 1300 N. Country Club Road, Garland, Texas.
