29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:  a) For employees working at Highway 70 West at approximately station number 697 in Arcadia, as observed on or about August 26, 2014. Employees were working in the excavation that was approximately 290 feet in length and only 2 means of egress were in use. Two ladders were in use for employees to egress from the excavation that were located approximately 140 feet apart. There were no ramps or other means of egress available on the excavation.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.   a) For employees working at Highway 70 West at approximately station number 697 in Arcadia, as observed on or about August 26, 2014. Employees were using a step ladder that was leaning against the edge of the excavation for access to the upper level landing surface. The top of the step ladder was flush with the upper landing surface and no grasping device was provided.

Employees were using a step ladder that was leaning against the edge of the excavation for access to the upper level landing surface. The step ladder was used in the closed position and was not used in the way it was designed to be used.

Employees were using a front end loader that had a cracked windshield. Operators view distorted by numerous longitude cracks in the glass.

Employees were working in an open excavation and were not protected from cave-in. The depth of the excavation averaged 5 feet 6 inches and the excavation was approximately 290 feet long by approximately 20 feet in width.
