29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.   On or about September 9, 2014, during the installation of roofing material on an approximately 3/12 pitch roof the employer did not ensure employees were protected from a fall of approximately 19 feet by utilizing a guardrail system, personal fall arrest system, or combination of a warning line system and safety monitoring system.         WOOSTER BUILDINGS, LLC WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD, 29 CFR 1926.501(B)(10), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 313785537, CITATION NUMBER 01, ITEM NUMBER 002, AND WAS AFFIRMED AS A FINAL ORDER ON APRIL 6, 2011, WITH RESPECT TO A WORK PLACE LOCATED AT 3510 CLEVELAND ROAD, HURON, OHIO.
