29 CFR 1910.215(b)(9): Guard for abrasive wheel machine where the operator stands in front of the machine was not constructed so that the peripheral protecting member could be adjusted to the constantly decreasing diameter of the wheel: On or about September 4, 2014, and at times prior thereto, the employer did not assure the abrasive wheel had an adjustable guard for employee to adjust as the wheel diameter decreased. Employee was exposed to hazards in the event the stone wheel exploded.

29 CFR 1910.304(b)(3)(ii)(C)(3): 29 CFR 1910.304(b)(3)(ii)(C)(3): Each cord set, attachment cap, plug, and receptacle of cord sets, and any equipment connected by cord and plug found damaged or defective was used before being repaired: On or about September 4, 2014, and at times prior thereto, the employer allowed the use of electrical equipment which had the outer insulation of the power cord pulled back from the electrical plug. The employee was exposed to the possibility of electrical shock.
