29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  Location:  58 Stannard Avenue, Branford, CT 06405    Each employee working on the aluminum scaffold planks spanning between two step ladders was not trained by a qualified person in the appropriate subject matter (scaffolding) to be able to recognized hazards and know what procedures to use to control or minimize those hazards.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:  Location:  58 Stannard Avenue, Branford, CT 06405    a) An 8 foot Louisville fiberglass step ladder was not fully extended and leaning against a wall with a scaffold plank resting on the sixth rung.    b) Two 8 foot Louisville fiberglass step ladders were used as a support (base) for a scaffold plank that rested on the sixth rung of each ladder.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards.  Location:  58 Stannard Avenue, Branford, CT 06405    Employees using Louisville fiberglass step ladders at a residential construction site for soffit installation were not given training in all OSHA requirements for proper use of portable ladders and procedures to be followed to minimize and correct hazards.
