29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), through (c)(4)(ii)(D):     a) International Timber and Veneer, L.L.C. - August 29, 2014, and times prior thereto, lockout/tagout procedures were not specific for machines and equipment that include but were not limited to the Slicers and Flitch Planer.  The current procedures were generalized and did not contain information including but not limited to:   (1) Specifically identify the energy sources and magnitudes of energy for each machine.  These energy sources for each machine that were not identified included but were not limited to hydraulics, pneumatics, and stored energy (potential energy).  (2) Specific identification of isolation points/locations for each energy source.  The specific energy isolation points/locations that were not identified were associated with energy sources that included but were not limited to hydraulics, pneumatics and stored energy (potential energy)  (3) Specific sequential techniques for bringing the machine to a zero energy state   (4) Specific techniques for verifying energy isolation   (5) Specific methods/techniques to dissipate/disable stored energy sources

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:    a)  International Timber and Veneer, L.L.C. - on or about August 29, 2014, and times prior thereto, employees conducted servicing and maintenance activities on machinery that included, but was not limited to, the slicers and flitch planer.  Periodic inspections of the lockout/tagout procedures had not been conducted.

29 CFR 1910.147(e)(3): Specific procedures for the removal of lockout devices by someone other than the employee who applied the lockout device were not developed, documented and incorporated into the employer's energy control program:    a) International Timber and Veneer, L.L.C. - August 29, 2014, and times prior thereto, specific procedures were not developed for the emergency removal of lockout devices by someone other than the employee who applied the lockout device.  The procedures included in the company's Lockout Tagout Procedure Guide were general and merely restated the requirements of the OSHA standard.  Employees conducted servicing and maintenance work on machinery/equipment that included but were not limited to Slicers and Flitch Planer.

The conduit had been pulled from the box, exposing the inner electrical wires.
