29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: (a) On or about August 27, 2014, employees in the production area of the facility were exposed to possible electrical shock and/or electrocution hazards in that a non-double insulated hand drill (used for mixing) lacked a path to ground due to its grounding pin having broken off of the drill's 3-wire male plug. 29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: (a) On or about August 27, 2014, employees in the production area of the facility were exposed to possible electrical shock hazards while using an electric hand drill to mix products. The strain reliefs on the drill's electrical cord were broken at both ends (i.e., at the male plug end, and the drill's handle) allowing strain to be transmitted directly to the internal conductors and their attachment points. Abatement certification is required for this Item. Use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
