29 CFR 1910.179(g)(1)(v): Pendant control box(es) were not clearly marked for identification of functions:   (a) On or about August 27, 2014, employees were exposed to possible struck-by or crushing-type injuries while using the overhead cranes to move loads in that some of the control markings on the cranes' pendant controls were either missing entirely or illegible.   Abatement certification is required for this Item.  Use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.  NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. 29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i.:   (a) On or about August 27, 2014, employees in the production area of the facility were using compressed air for cleaning purposes.  The nozzles were not vented and could achieve a dead-ended pressure well in excess of 30 p.s.i.   Abatement certification is required for this Item.  Use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.  NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. 29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective:   (a) On or about August 27, 2014, employees in the production area of the facility were exposed to possible electrical shock and/or electrocution hazards in that a non-double insulated hand drill (used for mixing) lacked a path to ground due to its grounding pin having broken off of the drill's 3-wire male plug.   Abatement certification is required for this Item.  Use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose. 29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:   (a) On or about August 27, 2014, employees in the production area of the facility were exposed to possible electrical shock hazards while using an electric hand drill to mix products.  The strain reliefs on the drill's electrical cord were broken at both ends (i.e., at the male plug end, and the drill's handle) allowing strain to be transmitted directly to the internal conductors and their attachment points.   Abatement certification is required for this Item.  Use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.  NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.
