29 CFR 1926.501(b)(14): The employer did not ensure that each employee working on, at, above, or near wall openings (including those with chutes attached) where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening was less than 39 inches (1.0 m) above the walking/working surface, was protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system:  a)  On the South End of the 3.5th floor - On or about September 4, 2014, employees, delivering and receiving drywall, were exposed to a fall hazard of approximately 34.6 feet when the 8-foot window opening was cut open with a box cutter from the 37-inch high windowsill.    b)  On the South End of the 3.5th floor - On or about September 4, 2014, employees, delivering and receiving drywall, were exposed to a fall hazard of approximately 34.6 feet when the guardrail was removed from the 37-inch high windowsill during the drywall delivery.    c)  On the South End of the 2.5th floor - On or about September 4, 2014, employees, delivering and receiving drywall, were exposed to a fall hazard of approximately 22.3 feet when the 8-foot window opening was cut open with a box cutter from the windowsill.    d)  On the South End of the 2.5th floor - On or about September 4, 2014, employees, delivering and receiving drywall, were exposed to a fall hazard of approximately 22.3 feet when the guardrail was removed from the windowsill during the drywall delivery.    Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
