29 CFR 1910.147(c)(6)(i): The employer did not conduct periodic inspections of the energy control procedures at least annually to ensure that the procedure and the requirement of this standard were being followed:    (a)CAPCO, Inc. at 1328 Winters Ave., Grand Junction, CO 81501:  On and before September 3rd, 2014, the employer did not conduct annual inspections of the energy control procedures at least annually to ensure that lockout tagout procedures were being followed.  The condition exposed employees to amputation and burn hazards.    Abatement note:  29 CFR 1910.147(c)(6)(ii) also provides that the employer shall certify that the periodic inspections have been performed.

29 CFR 1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not provided with electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed:    (a)CAPCO, Inc. at 1328 Winters Ave., Grand Junction, CO 81501:  On and before September 3rd, 2014, the employer did not ensure that maintenance employees wore appropriate electrical protective equipment when performing work on energized panels up to 208 volts, and when performing voltage testing on energized circuits up to 480 volts.  The employees performed this work without wearing:  1) rubber insulated gloves;  2) arc-rated long-sleeve shirts and pants (or coveralls) having an a minimum arc-rating of 8;  3)  arc-rated flash suit hoods, or arc-rated face shields and balaclava with a minimum arc-rating of 8.  Conditions exposed employees to electrical shock, arc flash, and arc blast hazards.    Abatement note:  NFPA 70E, 2012 Edition, Table 130.7(C)(15)(a) and Table 130.7(C)(16) categorize activities into risk groups, and describe the protective clothing and personal protective equipment associated for each group.  This criteria provides that if voltage testing were being performed on energized 480 volt circuits, the PPE should satisfy an arc-rating of 8 based on Hazard/Risk Category 2.  Table 130.7(C)(15)(a) also provides that rubber insulating gloves be worn for voltage testing both 208 and 480 volt circuits. Note that the purchase of equipment that is too heavy, such as arc-flash suits with an arc-rating of 32, may cause the employees not to wear it because it may become too hot, when gear with only an arc-rating of 8 is required.

29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated, the person certifying that the evaluation has been performed, the dates of the hazard assessment, and which identifies the document as a certification of hazard assessment:  (a)CAPCO, Inc. at 1328 Winters Ave., Grand Junction, CO 81501:  On and before September 3rd, 2014, the employer did not identify a document as a written hazard assessment and did not certify that a written hazard assessment was performed.  The condition exposed employees to physical injuries associated with the lack of PPE, including electrical shock and burn hazards.
