29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    (a)Escobedo Framing at 3932 Wild Elm Way, Fort Collins, CO 80528:  On and before August 30th, 2014, the employer did not ensure that employees wore safety glasses when using or working in the vicinity of pneumatic nail guns.  The condition exposed employees to the hazard of projectiles to the eyes.

The condition exposed employees to fall hazards of 40 inches.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected from falling by a guardrail system, safety net system, or a personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):    (a) Escobedo Framing at 3932 Wild Elm Way, Fort Collins, CO 80528:  On and before August 30th, 2014, the employer did not ensure that the employees were protected from falling when framing this house in the following instances:    1)Employees were framing a second floor wall on the second floor of the house over the second floor open stairway hole with no fall protection;  and    2)Employees were framing a second floor wall on the second floor of the house within 6 inches of the edge with no fall protection.    The condition exposed employees to fall hazards of 12.5 feet.

29 CFR 1926.502(i)(3): All covers were not secured when installed so as to prevent accidental displacement by the wind, equipment, or employees:  (a)Escobedo Framing at 3932 Wild Elm Way, Fort Collins, CO 80528:  On and before August 30th, 2014, the employer did not ensure that the cover over the stairway hole on the main floor, with the hole to the basement, was secured.  The condition exposed employees to a fall hazard of about 9 feet.  Abatement Note:  29 CFR 1926.502(I)(4) provides that the covers over holes shall be marked with the work "HOLE" or "COVER" to provide a warning of the hazard.

29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step:    (a)Escobedo Framing at 3932 Wild Elm Way, Fort Collins, CO 80528:  On and before August 30th, 2014, the employer allowed the employees to use the top and top step of a stepladder in the following instances:    1)Employees accessed the second floor using a 6 foot Werner stepladder, model NXT1AO6, with a distance of 52 inches from the second floor landing to the top, and 64 inches from the second floor landing to the top step; and    2)Employees accessed the second floor using an 8 foot Werner stepladder, model FS108, with a distance of 32 inches from the second floor landing to the top, and 44 inches from the second floor landing to the top step.    The condition exposed employees to fall hazards of 10 feet 3 inches.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:  (a)Escobedo Framing at 3932 Wild Elm Way, Fort Collins, CO 80528:  On and before August 30th, 2014, the employer allowed the employees to use an extension cord that was missing its ground plug to operate a 120 volt Dewalt circular saw, model DWE575, serial number 284099, that was double insulated.  The condition exposed employees to an electrical shock hazard.
