29 CFR 1910.23(a)(8)(ii): Every floor hole into which persons could accidently walk was not guarded by a floor hole cover of standard strength and construction.  (a) At the NPS South Rim Waste Water Treatment Office, there was a 13 inches long, 7 inches wide and 15 foot deep hole in the floor above the pump room.  (b) At the NPS South Rim Waste Water Treatment Head Works Bldg, there were many floor holes including a 9 inch long, 8 inches wide and 4 foot deep hole in the floor above the water raceway to the bar screen process.

(a) At the NPS South Rim waste Water Treatment Supervisor's office had materials laying on the floor that was blocking walkways and creating a tripping hazard.  (b) At the Reclaim Building Pump Room, there was water on the floor which created a slipping hazard.  The National Park Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.22(a)(1), which was contained in OSHA inspection number 315884049, citation number1, item number 1, and was affirmed as a final order on 12/16/2011 with respect to a workplace located at Haleakala National Park, Makawao, HI 96768.
