29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    (a)G&T Pipeline Construction, Inc. at Spring Gulch Road, 1/2 Mile West of H 103, Idaho Springs, CO 80452:  On and before August 29, 2014, the employer did not ensure that employees were protected from falling into the adjacent ravine through the use of guardrails, safety net systems, or personal fall arrest systems.  The condition exposed employees to a rolling fall hazard of approximately 25 feet on an approximately 70 degree slope.

29 CFR 1926.651(d): Employees exposed to public vehicular traffic shall be provided with, and shall wear, warning vests or other suitable garments marked with or made of reflectorized or high-visibility material:    (a)G&T Pipeline Construction, Inc. at Spring Gulch Road, 1/2 Mile West of H 103, Idaho Springs, CO 80452:  On and before August 29th, 2014, the employer did not ensure that employees were wearing reflective vests when working within a work zone. The condition exposed employees to struck-by hazards from vehicular traffic.

29 CFR 1926.651(j)(2):  Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary:    (a)G&T Pipeline Construction, Inc. at Spring Gulch Road, 1/2 Mile West of H 103, Idaho Springs, CO 80452:  On and before August 29th, 2014, the employer did not ensure that the spoil pile was at least 2 feet from the edge of the excavation.  The condition exposed an employee to cave-in hazards, and struck-by hazards resulting from rolling rocks and clumps of soil striking the employee.
